First you have to get an affidavit of correction/change of name attested by first class magistrate and then have to publish in a well known newspaper. Then with that affidavit correct your name in your birth certificate and  then you have to give an application to your school for correcting your name. If you have already filled up all forms of the education board then you have to apply to the board too for correcting the name.
