  1. Dear mahidul Islam

    You can change your name through Affidavit. And this affidavit must have to be notarized by 1st Class Magistrate. With this affidavit you can change your name in SSC, HSC, Honours, Masters Certificates, National ID Card and other documents. Moreover, you also can change name in your passport with this Affidavit. But office to office you will be needed other supportive documents as may be required by the concerned offices.

    If you need further assistance don't hesitate to ask further question.

    Thanks.

      • Do I need to specify a reason for name change?  I just want to drop "Md" from my name. If below statements would be enough to make this change-

        That my birth name is Md Jamal Ahmed;
        That by virtue of this affidavit, I changed my name from Md Jamal Ahmed to Jamal Ahmed and henceforth I shall be known as Jamal Ahmed for all purposes;
        That the facts stated above are true to the best of my knowledge and belief and in truth whereof I swear and sign this affidavit.

    • Dear M Tushar Ahmed,
      Dear Muhammad Tushar Ahmed,

      I am a Pupilage Lawyer and working in a Barrister Chamber, am advising you from my personal experiences RE “Name Change” only. Name change in UK is very strait-forward and easy, where a simple Notary public/commission for oath in Solicitor’s office is sufficient. It only cost me £120 in 1997. The name change “ Change of name Deed” is accepted everywhere i.e. educational institutes, Banks, Credit cards companies, Passport office, Employer or anywhere. Its one stop service and change it instantly.

      My personal experiences in Bangladesh both statutory and autonomous offices where name change authorities concern were slightly painful. Everywhere I had to go for name change then 1st question the office raised why I had to change my name after so long (since I have passed my 1st public exam i.e. SSC in 1996)? And why do you want to add your family name after so long (I have added “Mullah” as my family name)? Lastly your fathers’ family name hasn’t included in your docs so how you going to prove that that “Mullah” is your family name? I always carry a good answer with documents. I have managed them all changed within 3 months after hard work and help of my friends and relatives.

      Documents I produce for name change:

      1.    A duly-sworn affidavit setting forth the reasons for which the change is applied for on BDT 200.00 non-judicial stamps by Notary Public (Atleast 5 pcs as Passport office, education board, Public University etc. asked for genuine one and rest of offices accepted attested photocopy of Affidavit)
      2.    Foreign Affidavit that I have made name correction in UK
      3.    Evidence where “Name Change” accepted in Bangladesh. (Please keep a copy of NCC meeting notification and certificates, If Dhaka Education Board accepted then University of Dhaka normally accept)
      4.    My fathers Passport, NID, Matriculation certificate, Land deed (Dalil) and birth certificate where his father that is my grandfather’s name with family name as “Mullah”.  (All attested by 1st class gazetted officers)

      Public or Statutory or Autonomous offices that I have to in touch:

      1.    Passport Office:
      2.    NID Office:
      3.    BRTA:
      4.    Banks:
      5.    Divisional Education Board:
      6.    Public/ Private Universities:
      7.    Local Birth and death Registrar office:
      8.    Local Thana Election commission office:
      9.    Professional bodies i.e. Bangladesh Bar Council (BBC), Bangladesh Medical & Dental Council (BMDC), Pharmacy Council of Bangladesh, The Institute of Cost and Management Accountants of Bangladesh (ICMAB), The Institute of Chartered Accountants of Bangladesh (ICAB) The Institution of Engineers, Bangladesh (IEB) and The Bangladesh Press Council (BPC):
